Faculty Feedback Survey

When it comes to professional development at your school, your voice matters!

The following survey aims to get your perspective in three broad areas: instruction, school culture, and professional learning. Most of the questions ask you simply to indicate the degree to which you agree/disagree with the statements.

Please use the following scale:
1= strongly disagree, 2 = disagree, 3 = neither agree/disagree, 4 = agree, 5 = strongly agree, n/a= not applicable/no answer

This survey is entirely anonymous – your school administrators will receive only aggregated data, not specific survey responses. So, please be as candid and complete as possible in your responses (especially the “open response” questions – #15, #31, #50 and #51).

The survey should take about 15 minutes to complete.

Instructional Practices

1. My school has an articulated vision of a graduate’s knowledge and skills.
2. I have a clear understanding of what teaching excellence looks like at my school.
